DEADLINE:  Monday, July 31, 2017

CANCELLATIONS:  There will be a 50% penalty for registrations cancelled between
August 1 and August 31. No refunds after August 31.

Instructions

     1.  Have your court financial administrator e-mail Form AO213 to Registration Chairman, Debbie Nelson, at debbie_nelson@wawd.uscourts.gov. She will complete and return the form to your court.

     2.  Print out and complete the attached registration form.
            - Write in the purchase order number for your registration fee.
            - Register for the Friday Night Event, if you plan to attend.
            - Select your banquet meal.
            - Register your guest, if you are bringing your spouse or a family member.
            - Total the amount due for the above-referenced items and mail your
              check and the registration form to Debbie Nelson, as indicated.

     3.  Once you have mailed your registration form, you may make your hotel reservation.
